A partnership
Your enrolment signals the beginning of a two-way contract with The Hastings Health Centre. It outlines your and our rights and responsibilities, explaining that information needs to be provided both ways to ensure the partnership works.
You need to know about our services, relevant policies and procedures.  In turn, the practice team, taking responsibility for providing your ongoing health needs, requires your relevant health information/history.

Enrol now…

1. Complete an enrolment form
Come into the Health Centre or phone and request for an enrolment form to be posted out to you.  This includes a letter authorising your permission for The Hastings Health Centre to obtain health records from your previous practice, whether this is local or in another part of the country.  We will manage the transfer process for you, sending off the authorisation letter and arranging for your file to be confidentially transferred.  

2. Our credit policy
Our credit policy will be explained to you when you enrol and you will be asked to sign this policy confirming that you understand this.  Please contact us if you have any questions about this policy.  Automatic payments can be established if you prefer.

3. Choose your General Practitioner
Phone the main reception on 06 8738999 to find out which General Practitioners are registering new patients within their practice. You can then browse through our online information about each General Practitioner.

Your introductory consultation
It’s time to personally welcome you and answer any questions you may have.  Please bring along your completed questionnaire and any other health information.  This will be reviewed and you may be asked for additional information.
