We are enroling!

Dr Blaine Stride is joining the team at the Havelock North Health Centre from 2nd June. To enrol with Blaine, please follow the link below to complete an enrolment form. If you are already enrolled with a different provider at The Hastings Health Centre or Havelock North Health Centre, please email enrolments@hhc.co.nz with your full name, date of birth and NHI (if known) to request a transfer. 

Please note that whilst we will be working as fast as we can, experience in setting up new practices has taught us to anticipate delays with getting your enrolment processed. Some external factors in the transfer process are beyond our control, for example, gaining access to your medical notes from your current provider.  There may also be limited appointment availability through the initial start up  period.  In order to minimise any disruption to your current medical routine, please ensure you have adequate supply of your current medications. Once your enrolment is accepted, it will terminate your enrolment at your current practice.

By registering with us you gain automatic access to our quality healthcare and medical services as well as your own family doctor within one of our GP Suites at 303 St Aubyn Street Hastings or 26 Middle Road Havelock North.
